What are some clinics in San Jose, CA that will refer me for a medical marijuana card?

Im trying to find a clinic with MD's that will document my conditions (severe back pain and insomnia) on a medical marijuana referral form so I can apply for a medical marijuana card. Does anyone know of any in the area, or has anyone used a specific clinic in the area?
