How do I apply for a medical marijuana card?

I'm 18 and I live in Montana and want to get my medical marijuana card because I get extremely bad migraines and have panic attacks. I don't like the thought of taking potentially harmful prescriptions to help them. I'm having trouble finding out how to apply. Please don't leave any rude comments or I will report you. Thank you.
