Health Insurance Premiums to Rise with Health Reform

According to a Congressional Budget Office report, health insurance premiums would increase by as much as 13 percent for about 14 million Americans under the current US Senate health reform proposal. The Senate has claimed that its proposal would make health care more affordable.
The finding of higher health insurance premiums is in a report prepared by the Congressional Budget Office (CBO) and the staff of the Joint Committee on Taxation (JCT), which was done at the request of Senator Evan Bayh and many other senators. The senators had asked for the analysis to determine how health insurance premiums might be affected by enactment of the Patient Protection and Affordable Care Act, which was proposed by Senator Harry Reid on November 18, 2009.
According to the report, the higher insurance premiums would be paid by the 14 million Americans who need to buy their own health insurance policies and who make too much to qualify for proposed subsidies. Subsidies would reduce costs for an additional 18 million people. The report also notes that the 134 million people who are covered by a large employer may see their premiums stay the same or decrease by up to 3 percent.
The Senate's proposed health reform measure would impose a penalty of $750 per adult and $375 per child for people who refuse to purchase health insurance under the mandate. These penalties would take effect in 2016.
The proposed legislation would also impose several new fees on providers of health insurance and on manufacturers and importers of medical devices and brand-name prescription drugs. The CBO and JCT report estimates that these "would not result in measurably higher premiums for private coverage." The legislation would also reduce payments to hospitals and certain other Medicare providers, and significantly increase enrollment in Medicaid. These measures could cause health insurance premiums for private coverage to increase.
The findings of the CBO and JCT report, and especially the estimated higher health insurance premiums, will likely be taken up by Republicans as a tool to help block the proposed health reform measure. Senator Max Baucus (D-MT), chairman of the Senate Finance Committee, commented on the report, saying that "the vast majority of Americans will see lower premiums than they would if we don't pass health reform."

Subutex and Pregnancy? Do you have to increase your dose?

I've been taking Suboxone for a while now and am now taking 2mg and was about to start tapering when I found out I'm pregnant. My doctor switched me to Subutex and said I will have to increase my dose as I get further along. I really don't want to go back up since I was just about to try to taper off. Does anyone know if I can just stay at 2mg my whole pregnancy or even better still taper and be off completely???

ok so i detoxed yesterday, (water only) today i slipped up but i was sick because i ate too much like a fat pig, i didnt make myself sick it just came back up, in the end i managed to keep some of it down(:, tomorrow i'm fasting doing 30mins brisk walk round the park and monday i'm fasting and also going in the sauna's and on the treadmill brisk walking for 30mins, i'm going to take 2 multi vitamens each day and drink loads of water. how much quick water weight do you think i'll lose? don't tell me im stupid or i shouldnt do it, i want to and i know i can't live like this, its just a "quick fix" i'm 5ft 4 122lbs, female, 16 years old. how much im going to lose by tuesday? Doctors' Leaders Urge Politicians To 'Put Party Politics Aside' And Support The Alcohol Bill In Scotland

BMA Scotland welcomed the introduction of the Alcohol (Scotland) Bill, and called on politicians to put party politics aside and support the measures outlined in the legislation.
The BMA also expressed disappointment at the Labour Party's decision not to support minimum pricing, saying that it was "premature" and a "blot on their excellent public health track record".
In 2003, one in 20 deaths in Scotland was estimated to be attributable to alcohol. In 2007/08 there were 42,430 alcohol related discharges from general hospitals (1). Over the last 30 years, UK liver cirrhosis mortality has risen over 450% across the population (2), with a 52% increase in alcoholic liver disease between 1998 and 2002 (3). Scotland now has one of the highest cirrhosis mortality rates in Western Europe.

Dr Brian Keighley, Chairman of the BMA in Scotland, said:
"Scotland's alcohol consumption rates are staggering. More than a million people in Scotland are drinking hazardously or harmfully. There appears to be a view that drinking to excess is simply part of our culture - an inevitable national characteristic - so much so that we are ranked 8th in the world for alcohol consumption.
"Almost half of all deaths could be prevented by lower alcohol consumption. It is now our national duty to ensure that we do all we can to do what we can to tackle this epidemic.
"At the heart of this Bill are measures to stop the irresponsible pricing of alcohol driven by large supermarket chains who sell some of the strongest alcohol products at ridiculously cheap prices, often as a loss leader to attract customers.
"As a public health measure, minimum pricing can have a significant and positive impact on health. Research estimates a 40p minimum price, for example, can reduce alcohol-related deaths by 365 in the first decade and alcohol related hospital admissions by more than 6,000.
"Opponents to this legislation are concerned about a perceived impact of legislation on their business profits, yet they have failed to come up with any evidence that trade will be affected significantly. Doctors have no such agenda. We witness the harmful effects of alcohol misuse on our patients and their families every day and we believe that the measures outlined in this Bill offer a way out of this national epidemic.

Commenting on the Labour Party's decision not to support the Bill, Dr Keighley added:
"I am especially disappointed that the Labour group has decided not to support minimum pricing - a decision that will blot their excellent track record on public health policy. It is a premature step by the party in an attempt to derail this legislation before it's even begun. I am confounded by their announcement and I would urge them to consider, with an open mind, the evidence that will be presented at stage 1 of the Bill's process. By listening to the overwhelming evidence in support of minimum pricing, I believe that the Party's concerns could easily be resolved.

"I hope that politicians from all parties can put aside petty party politics and listen to the evidence that is presented during the progression of the bill. By supporting all the measures in this Bill Scotland can once again take its place as an international leader in public health policy."

silly i know BUT how much will i lose? no harshys.?

I've recently had a relapse on heroin which couldn't have come at a worse time for me as I've also recently completed my qualifications to become a personal trainer and finally get on with my life with a career that I truly enjoy and am passionate about. Considering installing a tankless, gas water heater system and underfloor heating--any experience with this?

We have an area to heat an a-frame which is approximately 750 sq feet. It will have to support one full bathroom and one guest bathroom as well as an apartment size clothes washer and a kitchen with hot water.

How do you know your turbos are failing in a 2001 Audi S4?

Okay so I got an RS4 clutch, new chip, miltek exhaust, new diverter valves, ghb blow off valves, and cold air intake. Everytime I gun it and it gets into higher revs it makes this reaaally annoying screeching noise like a dentists drill or something. I also noticed it makes the noise at higher revs when the gas tank is full. Anyone have any idea whats up?
Its been running well (74k miles), sometimes i just feel like i'm not getting the power I had expected.

Despite Gains, HIV/AIDS Remains Public-Health Priority, UNAIDS, WHO Say

News outlets continued to examine the 2009 AIDS epidemic update released Tuesday by the WHO and UNAIDS:
"The U.N. report said 'AIDS continues to be a major public-health priority' and called for more funds to support efforts to curb the epidemic and to distribute lifesaving drugs," the Wall Street Journal reports. "The U.N. report also suggested that health authorities need to focus resources on those most at risk" (Fairclough, 11/25).

Los Angeles Times: "About 4 million people were receiving AIDS drugs at the end of 2008, compared with 3 million the previous year. Nonetheless, an additional '5 million people need treatment and are not receiving it,' Dr. Teguest Guerma, acting director of the WHO's HIV/AIDS department, said at a Tuesday news conference. She said that about 2.9 million lives had been saved so far by increased access to the drugs as a result of the U.S. President's Emergency Plan for AIDS Relief and other international assistance programs" (Maugh, 11/25).

VOA News examines the U.S. contributions towards the global fight against HIV/AIDS. The article includes comments by Anthony Fauci, of the National Institute of Allergy and Infectious Diseases, and Michele Moloney-Kitts, assistant coordinator in the office of the global AIDS coordinator. According to Fauci, the National Institutes of Health have "spent about $42 billion from 1982 through fiscal year 2009 on HIV/AIDS research," funding Fauci credited to having helped improve the lives of people living with HIV/AIDS, the news service writes.
"Michele Moloney-Kitts … said the U.S. government to date has provided about $25 billion, making it the largest donor in the global fight against AIDS, tuberculosis and malaria." The article also includes Moloney-Kitts' comments about the future of PEPFAR under the Obama administration (Butty, 11/24).
"The update cautions that prevention programs often fail to target the populations that are most at risk," Science's "ScienceInsider" blog reports. "Specifically, stigma and local laws prevent many countries from tailoring prevention outreach to highly vulnerable groups like injecting drug users, men who have sex with men, commercial sex workers, and longterm couples in which only one partner is infected. And the sobering bottom line is that five people continue to be infected for every two who start treatment with anti-HIV drugs" (Cohen, 11/24).

Epidemics In Russia, Eastern Europe, China


RIA Novosti examines the findings of the report that "[o]ver 1% of Russian residents are HIV-positive," with the primary route of transmission being injecting drug use. "According to the report, about 37% of Russia's estimated 1.8 million drug users are HIV-infected. Young people account for a considerable number of infections among injecting drug users in the region," the news service writes (11/25).

KyivPost: "'With an adult HIV prevalence of 1.6%, Ukraine has the highest prevalence in all of Europe,' UNAIDS and WHO experts said." The report also found "the estimated number of adults and children living with HIV in Eastern Europe and Central Asia has grown by 66% to 1.5 million since 2001," according to the newspaper (11/25).
The Associated Press examines the report's findings that HIV "is now spreading fastest in China through heterosexual sex, a trend demanding new strategies to stave off a rebound in the epidemic after years of progress in containing it…" UNAIDS head Michel Sidibe said, "We are seeing a shift in the nature of the epidemic. … We need to ensure resource allocation is responding to that change."
The article includes details about how "[t]he government remains sensitive about [HIV/AIDS]" and stories of patients living with HIV/AIDS seeking additional support from the government (Kurtenbach, 11/25).

Xinhua/People's Daily Online also reports on the Chinese health minister's response to the UNAIDS report and highlights the efforts of the Chinese government to stop the spread of HIV/AIDS in the country (11/24). In a UNAIDS press release that lauded China's progress on HIV/AIDS Sibide's said, "The world eagerly anticipates China's enhanced role in global governance - and its leadership in the global response to AIDS" (11/24).

Young Drink And Drug Driving Shockingly High Finds Road Safety Week Report - Brake Pleads 'Not A Drop, Not A Drag', England

More than 1 in 4 young people (28%) admit to drink or drug driving, or both, or being a passenger with a young drink or drug driver, finds a shocking new report released at the start of Road Safety Week by Brake, the road safety charity behind the Week, in partnership with companies 3M and Alcosense.
The report, commissioned by the three agencies, also finds that:
FACT ONE: 1 in 6 young drivers admit to drink driving and 1 in 15 young drivers admit to drug driving - and often combine it with drink driving.
Out of 2,621 young drivers aged 17-25 interviewed for the report in schools and colleges, one in six (18%) admit to driving after drinking alcohol and one in 15 (7%) admit to driving on illegal drugs. Out of those who admit to drug driving, most (84%) say they drive on a cocktail of drink and drugs.
FACT TWO: 1 in 4 young people are risking their neck as a passenger with a young drink or drug driver.
The report also surveyed 4,143 young people in the same age bracket who travel as passengers with young drivers aged 17-25. Nearly one in four (23%) said they had been a passenger with a young drink or drug driver. (Drink and drug drive casualties are often passengers of young drivers.)
FACT THREE: Young people underestimate how long alcohol and drugs stay in their system, so may drive on drink or drugs unknowingly.
When asked questions about how long different alcoholic drinks (such as wine, beer, cider, spirits) or cannabis stays in your system, about half of young people underestimate the length of time.
FACT FOUR: 'Smashed and crashed and a young man'. If you admit to drink or drug driving you are also more likely to say you have crashed. You are also more likely to be male. A significant number of young men also believe they are better at handling drink and drugs and driving than women.
It's no surprise that the report finds a strong link between 'smashed and crashed' drivers; drivers who admit to drink or drug driving and who also say they have crashed while at the wheel. Four in ten young drivers who admit to driving on drink or drugs (39%) say they have also crashed, compared with one in ten young drivers who say they don't drink or take drugs and drive (10%).
The report also finds that a whopping three-quarters (73%) of young drivers who admit to driving smashed and having crashed are male.
A quarter of the young men (26%) surveyed believe that males are better than females at handling drink or drugs and believe they are therefore safer driving on drink or drugs.

New Data Emerges On Liver Transplant Survival Rates

Researchers at the University of Miami School of Medicine compared the outcomes of cirrhotic patients who underwent liver transplants for nonalcoholic steatohepatitis (NASH) versus alcoholic liver disease (ETOH) and found no statistically significant differences in post-transplant survival rates between the NASH and ETOH groups. Study findings are presented in the December issue of Liver Transplantation, a journal published by Wiley-Blackwell on behalf of the American Association for the Study of Liver Diseases.

According to the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), part of the National Institutes of Health, NASH affects 2 of Americans and ranks as one of the major causes of cirrhosis in the U.S. behind hepatitis C and alcoholic liver disease. Incidences of both NASH and nonalcoholic fatty liver disease (NAFLD) are increasing, possibly because of the greater number of Americans with obesity. Obesity also contributes to diabetes and high blood cholesterol, which can further complicate the health of someone with NASH. Because it is often a "silent" liver disease, NASH can lead to cirrhosis and permanent damage. Once serious scarring or cirrhosis is present, few treatments can halt the progression. Liver transplantation remains the only treatment for advanced cirrhosis with liver failure and is increasingly performed in people with NASH.

Vishal Bhagat, M.D, and colleagues demonstrated that liver transplant is an appropriate treatment for NASH patients, with survival rates comparable to those for ETOH patients. The team performed a retrospective chart review on all patients who underwent liver transplant for cryptogenic cirrhosis with NASH phenotype and alcoholic cirrhosis at the University of Miami from January 1997 through January 2007.

Cryptogenic cirrhosis was defined by absence of significant alcohol use (>20 gm/day), negative tests for viral hepatitis, negative autoimmune markers such as anti-nuclear antibody and anti-mitochondrial antibody, and negative markers for hemochromatosis, Wilson disease and alpha-1 antitrypsin deficiency.

The alcoholic cirrhosis (ETOH) group included patients who had history of significant alcohol, had no biochemical, serological and histological evidence of other known causes of cirrhosis, and underwent liver transplant from January 1997 through January 2007. Patients with hepatocellular carcinoma (HCC) and alcoholic cirrhosis with NASH phenotype were excluded from the ETOH group.

Baseline data on alcohol use, BMI, blood pressure and fasting serum levels of glucose, total cholesterol and triglycerides was recorded for both groups prior to liver transplant and after 6 months post-liver transplant. Demographic information was also collected including: recipient age, recipient sex, recipient ethnicity, donor age, cold ischemia time, biopsy proven acute and/or chronic rejection, biopsy proven recurrence of moderate to severe steatohepatitis in the transplanted liver, causes of retransplantation, graft survival, causes of death and patient survival.

"Our study presents the largest patient population with the longest follow-up published so far on the survival of patients with NASH cirrhosis," said Dr. Bhagat. "We found that although there was a trend towards lower patient survival in the NASH group compared with the ETOH group, the difference was not statistically significant."

In both the NASH and ETOH groups, sepsis (with or without multisystem organ failure) was the most common cause of death post-liver transplant, which is consistent with previously published data, followed by cardiovascular causes in the NASH group and malignancies in the ETOH group. Although higher number of patients died from cardiovascular causes in the NASH group (26%) compared with the ETOH group (7%), the difference did not reach statistical significance. "Cardiovascular mortality among NASH patients was much higher than published autopsy results in liver transplant patients and stresses the importance of control of the metabolic syndrome post-transplant in NASH patients," added Dr. Bhagat.

In a related study also published in Liver Transplantation, researchers at the Cleveland Clinic lead by Arthur McCullough, M.D., found that the development of metabolic syndrome following liver transplantation is on the rise and is a significant contributor to cardiovascular complications and morbidity and mortality. "Cardiovascular mortality and higher infection rates among patients with post-transplant metabolic syndrome remains a concern," said Dr. McCullough. "There is clearly a need for prospective studies to help identify and validate risk factors for post-transplant metabolic syndrome and thereby develop interventional strategies."

Alcohol In Pregnancy And Child Behaviour Problems Linked In New Study

A new study from Perth's Telethon Institute for Child Health Research has found evidence that the amount and timing of alcohol consumption in pregnancy affects child behaviour in different ways.

The study has just been published online in the international journal Addiction.

Lead author Colleen O'Leary said the analysis was drawn from a random sample of more than 2000 mothers who completed a questionnaire three months after the baby's delivery, and were then followed up when the child was 2, 5 and 8 years of age.

"Mothers who reported what we would classify as heavy drinking in the first trimester of pregnancy were nearly three times as likely to report that their child suffered with anxiety and/or depression or somatic complaints," Ms O'Leary said.

"Those who drank moderately during that first trimester were twice as likely to report those types of behavioural issues for their child.

"Exposure to moderate or heavy levels of alcohol in late pregnancy increased the risk of aggressive types of behaviours in the child.

"This research suggests that both the timing and the intensity of alcohol exposure in the womb affect the type of behaviour problems expressed."

In this study low levels of alcohol did not increase the risk of harm to the baby. However, the evidence clearly shows that the risk to the baby increases with increasing amounts consumed.

"It should also be noted that in this study moderate exposure is classified as drinking 3-4 standard drinks per occasion- that's about two normal glasses of wine-and no more than a bottle of wine drunk over a week."

Heavy drinking included women who were drinking the equivalent of more than a bottle of wine per week.

It is important that women who had consumed alcohol while pregnant are not panicked by the findings.

"Not every smoker gets lung cancer despite them being at higher risk - and in this case, not every child will be affected by prenatal exposure to alcohol. However it is important that women have this information about increased risk so that they can make informed decisions to give their child the best start to life," Ms O'Leary said.

The National Health and Medical Research Council recommend that the safest choice for women who are pregnant or planning a pregnancy is to abstain from alcohol.

Ms O'Leary said health professionals can assist by talking to women of child bearing age about their alcohol consumption and encouraging pregnant women and women planning a pregnancy to abstain from alcohol.

New York Times Magazine Examines Needle-Exchange Programs

In a New York Times magazine article, Tina Rosenberg examines how needle sharing has contributed to the HIV/AIDS epidemic and the viability of needle exchange programs as a prevention strategy. Rosenberg notes, "Drug injectors don't pass infection only among themselves. Through their sex partners, HIV is spread into the general population. In many countries, the HIV epidemic began among drug injectors. … Though it has been scorned as special treatment for a despised population, AIDS prevention for drug users is in fact crucial to preventing a wider epidemic."
She continues, "Unlike with sexual transmission, there is a proven solution here: needle-exchange programs, which provide drug injectors with clean needles, usually in return for their used ones. Needle exchange is the cornerstone of an approach known as harm reduction: making drug use less deadly." Rosenberg notes that clean needles also provide a means to connect drug users with HIV counseling, tests and treatment as well as drug rehabilitation programs.
"Needle exchange is AIDS prevention that works," Rosenberg writes, pointing to recent studies supporting this claim. "All over the world, however, solid evidence in support of needle exchange is trumped by its risky politics. Harm reduction is thought by politicians to muddy the message that drug use is bad; to have authorities handing out needles puts an official stamp of approval on dangerous behavior." With government funding of needle exchange "politically unpopular," Rosenberg says that programs are often operated by non-governmental organizations and "efforts are small, isolated and often undermined by uncooperative police and health departments."
Rosenberg suggests that "Russia needs needle exchange more than any other country: its HIV epidemic is large, one of the fastest-growing in the world, and perhaps the most dominated by injecting drug use" (11/17).

California Issues Strict Rules For Health Professionals With Addiction Problems

News outlets report on a variety of health issues at the state level including stricter rules for drug abusers in the health industry in California, an examination of the nation's first city-run universal health care plan in San Francisco, a cigarette tax in Florida, sex offenders and ex-convicts in nursing homes in Illinois and efforts to battle medical errors in New Jersey. The Los Angeles Times: "Nurses, dentists and other professionals with addictions will be subject to more drug tests, and any restrictions to their licenses will be listed on public websites. In a major shift, California will impose tough new standards on drug-abusing health professionals, strictly scrutinizing those in treatment and immediately removing from practice anyone who relapses. ... The rules will require nurses, dentists and other health workers in state-run recovery programs to take at least 104 drug tests in their first year -- more than double any current requirement. Health professionals wi
ll be automatically pulled from practice, at least temporarily, after a single positive result" (Ornstein and Weber, 11/20). The Associated Press: "Healthy San Francisco is the nation's first city-run universal health care plan. While not insurance and not valid outside the city, it does illustrate how some hotly debated elements of plans being considered on Capitol Hill might play out. In just over two years, the $126 million program has won over its target population, and now covers about 48,000 people - more than two-thirds of San Franciscans who previously had no insurance. About $20 million a year comes from employers who are required to contribute to their workers' health care. The mandate is a contentious aspect of the Democratic plan, and the most controversial aspect of San Francisco's plan" (Barbassa, 11/20). The Miami Herald: "A new tobacco tax is doing just what its proponents envisioned: reducing cigarette sales while fattening state coffers. Cigarettes sales a

But despite the drop in sales, tobacco-tax collections in Florida are high and holding steady. Amid reports that elderly and disabled residents were being assaulted and raped in nursing facilities, a state police unit in 2001 began raiding the homes to haul out unregistered sex offenders and ex-convicts with outstanding arrest warrants for crimes rangi
ng from armed robbery to murder. ... From January 2005 through June 2006, when 20 northern Illinois nursing homes were swept and roughly 80 fugitives and sex offenders removed, state police in that area recorded a nearly 67 percent decrease in nursing home abuse and neglect complaints, according to a department citation issued to the sweeps unit. Then the cheering stopped. In 2006, the program was halted after five years" (Jackson, 11/20). Asbury Park Press reports on the 10-year anniversary of "To Err Is Human," a report by the National Institute of Medicine that detailed the number of deaths caused by medical errors. "A decade later, New Jersey's hospitals and health care providers mark the report's 10-year anniversary with a growing list of accomplishments. Among them: In 2000, New Jersey ranked 49th in the nation in quality data reported by the federal Medicare program. Coming into contact with wedding vendors may be welcoming to the H1N1 virus, which could make one sick during a precious planning time. Take precautions to stop it.

So many things can interfere with wedding plans – including illness. With threats of the H1N1 virus rapidly being spread through a variety of human contacts, it is normal to fear catching it while planning a wedding. There are so many vendors to see, hands to shake, stores to visit and so much more. All of these could expose you to this season’s flu bug concern.
So how can the H1N1 virus be stopped while planning a wedding? Easily enough, as long as all precautionary measures are taken.
Keep hands clean
When going from vendor to vendor make sure to keep hands clean and sanitized. Shaking hands or touching something tath hs been handled by someone who has been in contact with the H1N1 virus may spread the virus. Wash hands thoroughly with soap and warm water after each vendor meeting throughout the day. Alcohol-based hand sanitizers are also a great prevention tool. Using both will help reduce chances of getting sick.
If a major phobia of contracting the H1N1 virus will be an issue, just let wedding vendors know and they may understand enough not to force a handshake. They may even help relax fear by explaining precautionary measures they are taking to prevent the spread of the virus.
Keep surrounding area clean
When shopping for wedding items make sure to keep surfaces clean. When going from vendor to vendor viruses may be carried into a car or house. Use alcohol-based sanitizer regularly on steering wheel, seatbelts, around the house and other frequent contact areas.
Also, shopping baskets and carts are a haven for viruses. Carry hand sanitizing wipes and use them to wipe down surfaces that may have been handled by previous shoppers.
Do not spread H1N1
To help prevent further spread of viruses, including the H1N1, make sure to cover mouth and nose when sneezing and coughing. Or better yet, use a tissue and toss it immediately.
Stay home if signs of illness appear while planning a wedding. It will be better to take a day off than to get others sick or be sick longer than expected. A sick vendor could delay wedding plans. A sick bride may have to postpone the ceremony.
Taking all precautions will protect the health of the bride, groom and anyone else involved in the wedding process. It is better to know all measures are being taken to prevent the spread of the H1N1 virus while planning a wedding than to become sick from not being 100 percent sure. No honeymoon is fun when one is sick.

Health Insurance Reform Could Harm Children

Concerns over the negative impact of health insurance reform on children have been raised recently by child health and industry advocates. One concern is the possible burden on children's hospitals because of cuts to hospitals, while another is associated with the repeal of the Children's Health Insurance Program (CHIP).
Reductions to Children's Hospitals
The first issue is the $155 billion reduction in federal payments over 10 years which hospital groups agreed to in July. A USA Today (Nov. 6) story reported that the industry's trade group (National Association of Children's Hospitals) claims these cuts will have a disproportionate impact on children's hospitals, resulting in significant reductions in the health services they will be able to provide for children.
The problem is the proposed reduction in federal payments that reimburse hospitals for the care they provide to people who have no health insurance or who are on Medicaid. Children's hospitals provide services to a greater share of patients on Medicaid than to general hospitals, and Medicaid pays hospitals less than private insurance. Children's hospitals also have less to gain from an increase in the number of people who will get health insurance under the reform package because most of them already have coverage.
Therefore, hospitals that serve children are lobbying Congress to stop changes to health care that they claim could cost them hundreds of millions of dollars. The industry trade group estimates legislation approved by the Senate Finance Committee would reduce funding to children's hospitals by $876 million. The bill in the House would cut it by $395 million.
Elimination of CHIP
Elimination of the Children's Health Insurance Program (CHIP) is another concern. The health reform bill passed by the House vote would repeal CHIP in 2014 and shift some low-income children into the Medicaid program while others would have to go to private plans that could be most costly and guarantee fewer benefits, noted The Washington Independent.
The reform bill passed by the House expands Medicaid eligibility to 150 percent of poverty and then sends all other children who live above that level to private health insurance plans, the so-called exchange mentioned in the reform package. While Medicaid programs are required to offer preventive care (a plan that includes early periodic screening, diagnosis and treatment), the exchange plans do not operate under the same mandate. Thus the families of children who end up with the exchange option will be forced to pay more for less coverage.
Not everyone is behind the drive to end CHIP. Sen. Jay Rockefeller (D-WVa) has sponsored an amendment to reauthorize CHIP through 2019, and members of the Finance Committee passed the amendment last month.
The future of health insurance reform in America is uncertain. For children who have no direct voice in the matter, the fallout of cuts to health insurance coverage and medical care could have a lasting negative impact on the rest of their lives.

Older Problem Drinkers Use More Alcohol Than Do Their Younger Counterparts

Older adults who have alcohol dependence problems drink significantly more than do younger adults who have similar problems, a new study has found.
The findings suggest that older problem drinkers may have developed a tolerance for alcohol and need to drink even more than younger abusers to achieve the effects they seek.
Researchers at Ohio State University found that adults over age 60 who have alcohol dependence drink more than 40 alcoholic drinks a week on average, compared to between 25 and 35 drinks a week on average for those in younger age groups with similar problems.
In addition, older people with alcohol dependence have more binge drinking episodes per month than do their younger counterparts.
"A combination of high levels of drinking and the physiological effects of aging are particularly problematic for older adults," said Linda Ginzer, co-author of the study and a doctoral student in social work at Ohio State.
Ginzer, who conducted the research as part of her dissertation, did the study with Virginia Richardson, professor of social work at Ohio State.
They presented their results November 20 in Atlanta at a meeting of the Gerontological Society of America.
The researchers used data collected in the National Epidemiologic Survey on Alcohol and Related Conditions. This was a national survey of more than 43,000 people collected in 2000-01 under the direction of the National Institute on Alcohol Abuse and Alcoholism.
Research has shown that Americans generally tend to drink less alcohol as they age. But these findings suggest that for certain groups of older adults - those with alcohol problems - alcohol use actually increases, Ginzer said.
For this study, the researchers used the survey results to classify heavy drinkers by age categories.
Two categories were of particular interest to the researchers. Those in the alcohol abuse category were those who showed mainly social-related problems related to their alcohol use, including legal issues, and engaging in physically hazardous activities such as driving after drinking. Those in the alcohol dependence category showed evidence of physiological problems related to their alcohol use, such as increasing drinking and continued use even after physical or psychological problems were apparent.
While adults over age 60 were less likely than other groups to be in the abuse or dependence categories, those who were in those categories tended to have higher drinking levels than did younger problem drinkers.
For one, older problem drinkers drank more each week than did others. In addition, older people in the dependence category had significantly more alcohol binges each month than did younger people in the same category. Binges were defined as men having five or more drinks in a day, or women having four or more drinks in a day.
Those over age 60 in the alcohol dependence category averaged 19 binges per month, while younger age groups in the same category averaged 13 to 15 monthly binges.
"More often than not, we think of binge drinking as occurring among college students or those in their 20s," Richardson said.
"But the fact is, binge drinking occurs among older people as well, and it is in fact worse among those who have problems with alcohol. It is something that clinicians and researchers need to consider."
Overall, binge drinking was greater among all adults who were in the alcohol abuse category than it was among other adults who reported drinking seven or more drinks a week, but did not fall into the problem categories.
"That suggests binge drinking may be a better measure of problem drinking than just the total amount of drinks someone has per week," Ginzer said.

How to Curl Your Lucky Bamboo

Curled lucky bamboo�is a very aesthetically pleasing plant that goes great in any home or office. Unfortunately, though straight plants are common, curled ones are rare and expensive. However, if you are on a budget—or if you like a challenge—you can curl lucky bamboo yourself with no special equipment. Here are some instructions that should help you if you decide to curl your lucky bamboo.
Materials

Lucky bamboo
Cardboard box
Artificial light (optional)
Scissors

Step 1 - Setting Up
Unfortunately, it is not possible to curl a mature growth of lucky bamboo. For this reason, it is vital to start with a young, green, growing lucky bamboo plant.
Select a cardboard box that is much bigger than your plant. The box needs to block all light from reaching your plant from most directions, even as it continues to grow.
Figure out a place where you can leave your bamboo plant inside the box. This place should have access to light, either artificial or natural.
Step 2 - Building a Container
Cut any flaps off of your cardboard box. At this point, your cardboard box should have pnly one side open. Turn it on its side so the open side faces your light source. Place your lucky bamboo upright in the box, but keep it out of direct sunlight.
You are now done, more or less. However, you will still need to take care of your lucky bamboo plant and make occasional adjustments to your setup.
Step 3 - Maintenance
Your bamboo plant will now grow normally, assuming you take good care of it. However, it will slowly curve towards the light source as it grows. Wait until the curve is noticeable. Then, rotate the pot slightly, and write down which direction and how far you turned it.
Wait some more. Once your bamboo curves towards the light again, rotate the pot again, in the same direction by the same increment. Repeat for as long as necessary. Be patient, as this can take a very long time
Step 4 - Advanced Tricks
Though curling lucky bamboo plants is a long, involved process, it is not actually much more labor intensive than taking care of these plants normally. If you are in the mood for a challenge, you should treat your bamboo plants as a sculpture.

Linking The Treatment Of Alcohol-Use Disorders And Tuberculosis

The integration of alcohol screening, treatment and referral into primary care and other medical settings is not routinely done. Nor are there any studies evaluating the effectiveness of integrating care for alcohol use disorders (AUDs) into routine treatment for tuberculosis (TB), despite the high co-occurrence and mortality associated with these two diseases. Accordingly, researchers have designed a trial study to determine the effectiveness of integrating pharmacotherapy and behavioral treatments for AUDs into routine care for TB.

The study will be published in the February 2010 issue of Alcoholism: Clinical & Experimental Research.

"In many primary-care settings, screening for drinking problems is not necessarily a routine part of visits," said Shelly F. Greenfield, director of clinical and health services research and education in the Alcohol and Drug Abuse Treatment Program at McLean Hospital and corresponding author for the study. "In many specialized medical settings, screening for alcohol problems is even less frequent," she added. "For example, in clinics that treat TB, it would not be common practice to screen for alcohol problems, yet alcohol problems often co-occur among patients with TB."

Greenfield said she and her colleagues chose to conduct their study in Tomsk, Russia because of its high rates of both alcohol problems and tuberculosis. "In Tomsk, we found that alcohol disorders in this population are common, with 50 percent of all TB patients meeting criteria for alcohol abuse or dependence sometime during their lifetime."

"This study is particularly important for Russia where multi-drug resistant TB is common," observed George Woody, a professor in the department of psychiatry at the University of Pennsylvania and Treatment Research Institute, "and where medical specialties are divided into silos that require patients to visit multiple providers for problems that are closely linked and could be better managed by integrated approaches like the one the authors are testing."

"Alcohol disorders not only place individuals at increased risk for acquiring a number of diseases, but once people acquire a disease like TB, alcohol places them at higher risk for poor outcome and death," added Greenfield. "This is because alcohol can suppress the immune system, alcohol hepatitis can complicate TB treatment since many TB medications are potentially toxic for the liver, and people with drinking problems are also less likely to be able to adhere to their TB treatment."

As part of the Integrated Management of Physician-delivered Alcohol Care for Tuberculosis (IMPACT) trial, Greenfield and her colleagues divided 200 patients - confirmed to have alcohol abuse or dependence, newly diagnosed with TB, and initiating treatment in the Tomsk Oblast Tuberculosis Service - into one of four groups: 1) oral naltrexone + brief behavioral compliance enhancement therapy (BBCET) + treatment as usual (TAU); 2) brief counseling intervention (BCI) + TAU; 3) naltrexone + BBCET + BCI + TAU; and 4) TAU alone. The trial is ongoing.

"To our knowledge, this is the first study to examine the feasibility of delivering alcohol treatment as part of routine TB care," said Greenfield, "and to assess this treatment model's impact on both TB and alcohol outcomes. If proven feasible and effective, this treatment model could be adapted for patients with AUDs and co-occurring medical conditions in other settings, specifically, anywhere co-occurring AUDs adversely affect TB outcomes, including the United States."

"The idea of integrated treatments is not new, but only just beginning to be studied and implemented in Russia and other countries, including the U.S.," said Woody. "These findings show the universality, practicality, and potential importance of developing integrated treatments for alcoholism, HIV, and other serious health problems. They could be life-saving not only for the affected individuals, but also for the community that is at risk for the spread of TB via cases that are inadequately treated due to poor adherence to prescribed medication. Finally, these findings could easily be applied, would likely be cost-effective, and could contribute to current efforts to reduce overall medical treatment costs."
