Four Section 1 items noted on the termite inspection report (details as quoted),how serious the problems are?

I quote from the Termite report:

Item 1: "Cellulose Debris (wood, paper products) with evidence of subterranean termites was noted in the substructure areas."

Item 2: Drywood termites were noted to Attic.

Item 3: Evidence of Drywood termite pellets noted to Attic.

Item 4: Fungus has damaged the underlayment/subflooring to the bathroom floor. Recommend to remove the floor covering and underlayment for further inspection

Also, according to the termite report, the proposed costs are:
Item 1: 5
Item 2: 5
Item 3:
Item 4: 5
